In any business or an organization, a procedure for incident
reporting and investigation to notify the work place incidents occurred during
the working hours should be one of the mandatory procedures.
The aim for this procedure is to maintain a safe and healthy
environment at the work place by investigating the occurrence of the incident
and taking a corrective action in order to mitigate the occurrence of such
event. It is also important to notify the management about the incident.

An incident
is an instance or unexpected occurrence which results in injury, illness,
damage to the people, property or environment.
These
incidents are categorized into minor, near miss incidents or major.

Incident Investigation and Reporting?
It is important to report the incident regardless of the type
of the severity of the incident (minor, near miss or major) and carry out the
investigation to understand the root cause of the occurrence of the incident and
the potential hazards that should be corrected.
A corrective action plan including the risk control measures
should be made to mitigate the re-occurrence of the similar type of incidents.
The investigation reports should be documented for future
references.
The effective measures should be implemented and the measures
should be evaluated in order to understand the effectiveness of the same.
These measures should be monitored and the required changes
